1. Just click on the "Online Enrollment Application" link below.
2.Carefully read and complete each section of the application.
Although some questions may seem inapplicable just answer them anyway.
3. Make sure that the person completing the application has educational rights for the enrolling student .
Once you have enrolled the student please make an appointment to visit your Sequoia Choice site and turn in any applicable documentation that may be missing such as:
- Birth Certificate
- Immunization Records
- Transcripts
- Withdrawal Slips
