Enroll online now by following these five simple instructions:

Make sure that the person completing the application has educational rights for the enrolling student.
- Carefully read and complete each section of the Enrollment Application and PHLOTE Form (Primary Home Language Other Than English). Although some questions may seem inapplicable just answer them anyway.
- Please complete the online Enrollment Application. [English]
- Please complete the online PHLOTE Form. [English] [Spanish]
- Please complete the online McKinney-Vento Form [English]
If you would rather apply offline please follow these eight simple instructions:

- Download and print out the Enrollment Application. [English]
- Download and print out the PHLOTE Form (Primary Home Language Other Than English). [English] [Spanish]
- Download and print out the McKinney-Vento Form [English]
- Carefully read and complete each section of the Enrollment Application and PHLOTE Form. Although some questions may seem inapplicable just answer them anyway.
- Please complete the printed out Enrollment Application.
- Please complete the printed out PHLOTE Form.
- Please fax both the Enrollment Application and PHLOTE Form to

Once you have enrolled the student please make an appointment to visit your Sequoia Choice site and turn in any applicable documentation that may be missing such as:
- Birth Certificate
- Immunization Records
- Transcripts
- Withdrawal Slips
